Job Mission:

• Deliver fundamental care services, including hygiene, comfort, and pressure ulcer prevention to ensure patients' well-being.
• Administer prescribed treatments, prepare and distribute medications, and closely monitor patients' conditions to provide optimal care.
• Evaluate and address malnutrition, assess and maintain elderly individuals' autonomy to promote independence.
• Offer palliative care support and collaborate with healthcare professionals to prevent falls and manage pain effectively.

Collaboration and Teamwork:

• Support families and work collaboratively with multidisciplinary teams to provide comprehensive care.
• Rehabilitate patients for independent living and coordinate with other healthcare professionals to ensure seamless care transitions.
• Communicate effectively with nursing homes and provide psychological support to patients' families to address their needs.
• Liaise with home care services to facilitate patients' return home and ensure a smooth transition.

Required Profile:

• Registered Nurse with experience in care services or a similar setting, possessing a strong understanding of patient care and management.
• Ability to work with diverse pathologies and individuals, demonstrating empathy and a patient-centered approach.
• Demonstrate autonomy, responsibility, teamwork, organization, and prioritization skills to excel in this role.
