Key Responsibilities- Deliver personalized care and support to patients in their homes, promoting independence and dignity.
- Conduct thorough assessments and develop individualized care plans to meet patients' unique needs.
- Administer medications, perform wound care, and provide other essential healthcare services as needed.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and communication.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
- DE Infirmier / Infirmière degree or equivalent.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage time efficiently.
- Ability to work independently and as part of a team, with a focus on delivering high-quality patient care.
- Valid Permis B and ability to travel to patient locations as needed.
