Position Overview

We are seeking a compassionate and experienced Registered Nurse to join our Alternative to Incarceration through Housing and Intensive Monitoring (AiLSi) service. This innovative program provides intensive support and care coordination for individuals with mental health disorders who are homeless and facing imminent court appearances.

Responsibilities

• Implement individualized social and housing plans, ensuring a smooth transition to stable living environments.
• Provide direct support and assistance to individuals, addressing their diverse needs and challenges.
• Facilitate successful adaptation to new living arrangements, fostering a sense of belonging and community.
• Collaborate with healthcare providers to ensure comprehensive social integration and well-being.
• Conduct home visits and follow-up sessions, monitoring progress and providing ongoing support.
• Manage addiction care services, connecting individuals with appropriate resources and treatment options.

Qualifications

• State Registered Nurse diploma required.
• Experience working with individuals experiencing homelessness and mental health challenges is highly preferred.
• Understanding of the legal system and prison procedures is beneficial.
• Knowledge of innovative intervention methods, such as recovery-oriented care, is an asset.
• Valid driver's license and personal vehicle are essential for conducting home visits.
