Ergothérapeute A4+

Job Summary

UGECAM ILE-DE-FRANCE is seeking an experienced Ergothérapeute A4+ to join our team in the field of autonomy and rehabilitation. The successful candidate will be responsible for evaluating and accompanying individuals with disabilities, aged 16 and above, to prevent or exit situations of rupture of care pathways and participate in the coordination of their socio-professional inclusion journeys.

• Evaluation of Autonomy
  • Collect information related to the intervention context and the individual's data (capacities, environment, habits...)
  • Put the individual in practical situations (apartment, public transportation, etc.)
  • Evaluate and describe the individual's level of independence and autonomy (PAAC, Profil des AVQ)
• Diagnostic Ergothérapeutique
  • Analyze data and draw conclusions
  • Develop recommendations for the individual and their entourage
  • Communicate results to the multidisciplinary team (written transmissions, synthesis meetings)
  • Participate in the development and evolution of the Individualized Plan for Care (DIPC)
• Rehabilitation and Accompaniment
  • Develop personalized objectives based on identified needs and individual expectations
  • Implement personalized compensation means for daily life activities
  • Inform and sensitize the individual, their entourage, and professionals about the situation of disability and the use of compensation means
• Coordination of Parcours
  • Conduct welcoming interviews to identify individual needs and expectations
  • Develop recommendations in line with individual requests or reorient
  • Implement recommendations
• Communication and Data Transmission
  • Realize targeted written transmissions, participate in meetings, and write reports
• Networking
  • Actively maintain relationships with partners (MDPH, ESAT...)

• Education
  • Diploma in Ergotherapy
• Skills
  • Knowledge of the healthcare and social environment
  • Understanding of the global situation of the individual and the specifics of the disability situation
  • Synthesis skills
  • Written and oral communication skills
  • Initiative and adaptability skills

Remuneration

Niveau 6E CCN, salary from 32K€ per year, negotiable based on profile, plus prime ségur € per year

Physical Constraints

• Frequent travel throughout the department
• Many tasks outside of face-to-face interactions
