As a **nurse** in a **rehabilitation and care facility**, your primary responsibilities will include a range of essential care tasks:
- Ensuring **hygiene**, providing **comfort care**, and preventing **pressure ulcers**
- Administering **prescribed treatments**, preparing, dispensing, and monitoring **medications**
- Conducting **nutrition assessments** and managing dietary needs
- Evaluating and supporting the **autonomy** of elderly patients
- Providing assistance in **palliative care**
- Collaborating with the team to prevent **fall risks** (through environmental adjustments and provision of suitable equipment)
- Assessing **pain levels** and implementing appropriate protocols
You will work closely with **multidisciplinary teams** in the unit through various missions:
- Supporting **family relations** and providing necessary assistance
- Collaborating with medical and paramedical **multidisciplinary teams**
- Facilitating **patient rehabilitation** to promote autonomy in preparation for discharge, in partnership with other professionals (such as physiotherapists, occupational therapists, psychologists, and social workers)
- Communicating with professionals from nursing homes regarding potential **institutionalization**, as well as providing information and psychological support to the patient's family
- Coordinating with home care organizations for patient discharge (including home nurses and home care services)
