GAPAS is dedicated to providing a range of assistance for individuals with disabilities, including both children and adults, through the management of numerous social and medico-social facilities across the Hauts-de-France and Ile-de-France regions. The organization is committed to fostering solutions that enable individuals with disabilities to thrive in a fair, inclusive, and equitable society, ensuring they are treated as full citizens. GAPAS champions the self-determination of individuals with disabilities and advocates for comprehensive support for all.
Position Overview
The services offered by GAPAS for children with disabilities in the Hauts de France region encompass various establishments and services categorized under the 0-25 Sector.
The 0-25 sector serves individuals aged 0 to 25 and includes:
✓ Three medico-social establishments (two motor education institutes and one medico-educational institute) providing both boarding and day care services.
✓ A collection of services (an early medical-social action center, a family support and early education service, and two services focused on promoting autonomy and education); these services aim to support the child's living environment and family.
All facilities are situated within the Lille metropolitan area, ensuring proximity with a maximum distance of 16 km between them. In summary, the 0-25 sector encompasses:
✓ 381 children receiving support; ✓ Ages ranging from 0 to 20, with provisions for those over 20 under specific amendments; ✓ Individuals with disabilities; ✓ A variety of support options, from low to high intensity; ✓ Regional intervention territory (Hauts de France); ✓ Funded by ARS Hauts de France and the Departmental Council (CAMPS).The 'La Source' site provides care for 20 children aged 3 to 10 in an external setting, while the 'Le Passage' site supports 32 young individuals aged 10 to 20, including 12 in weekday boarding.
The 'La Source - Le Passage' IEM caters to young people with complex disabilities, including motor disabilities, intellectual challenges, and sensory impairments.
Key Responsibilities
Reporting and Functional Relationships:
The social support specialist reports directly to the deputy director and collaborates functionally with GAPAS professionals, the service team, and all individuals involved in the daily care of the child.
Location:
IEM 'Le Passage and La Source' and all common law and inclusion locations for the child (daycare, cultural and leisure centers, sports venues, school, UEE)
Possible travel to GAPAS establishments
Duties:
In alignment with the establishment's project, the social support specialist is empowered to assess needs, develop, and implement individualized projects for young children. The specialist acknowledges and leverages the expertise of caregivers, focusing on consistency in care.
Specific tasks include:
➢ Establishing a strong socio-educational rapport with young individuals ➢ Engaging in educational support during daily activities (including separation, nursing care, toilet training, mobility acquisition, communication, meal independence, and exposure to the external environment). ➢ Coordinating and facilitating recreational, educational, cultural, sports, and leisure activities essential for achieving the objectives outlined in the individual project, while adhering to health authority guidelines to enhance the emotional, cognitive, physical, artistic, and social skills of young children. ➢ Collaborating with multidisciplinary team members and external partners involved in the child's care (daycare, leisure centers, school, ESAT, Impro) ➢ Considering all relevant care project information that may influence educational activities and actively participating in its execution ➢ Communicating observations and practices through team interactions ➢ Finalizing the design and documentation of individual projects in close collaboration with the educational referent and ensuring their implementation ➢ Contributing to written entries in family communication logs ➢ Reflecting on and adjusting practices, critically analyzing ethical dimensions, and participating in team practice reviews, with a focus on institutional and educational values ➢ Supervising and mentoring trainees as needed ➢ Engaging in reflection, research, and professional developmentIn a coordination role:
➢ Acting as the IMAGO referent for IEMs, managing software administration, including account creation and updates, dedicating time weekly to this role. ➢ Assisting colleagues in updating administrative data for young individuals ➢ Ensuring timely updates of each young person's individual projects within the IEMs ➢ Supporting the development of Augmentative and Alternative Communication (AAC) within IEMs ➢ Reporting on activities and support provided to management when requested ➢ Proactively engaging in project initiativesIn projects related to pre-professionalization and daily life autonomy/social skills:
➢ Coordinating activity schedules for adolescents transitioning from school projects and young adults engaging in pre-professionalization actions. ➢ Establishing partnerships with improv groups, ESATs, and volunteer organizations ➢ Supporting young individuals in identified projects ➢ Promoting autonomy in various city services for youth ➢ Organizing activities related to daily life autonomyCandidate Profile
