Care Coordinator

We are seeking a highly skilled and compassionate LPN Medication Care Manager to join our team at Sunrise Senior Living. As a key member of our care team, you will be responsible for providing exceptional care and services to our residents, ensuring their physical and emotional well-being.
Key Responsibilities- Administer medications and treatments in a safe and organized manner
- Document medication administration and maintain accurate records
- Conduct Service and Health Updates as directed by RCD
- Participate in the development of Individualized Service Plans (ISP)/Individualized Care Plans (ICP) and monthly updates
- Partner with community team to ensure compliance with OSHA/British Columbia Workers Compensation Act and Occupational Health and Safety Regulation requirements
- Practice safety procedures at all times, including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures
- Previous experience working with seniors preferred
- Desire to serve and care for seniors
- Ability to react and remain calm in difficult situations
