Position Overview:
We are seeking a dedicated professional to join our team as a Social Care Assistant. This role is essential in providing support and guidance to individuals and families in need.
Key Responsibilities:
- Conduct thorough social evaluations and assist in the formulation of care strategies in collaboration with a diverse team of professionals.
- Guide families in navigating their entitlements and resolving administrative challenges.
- Facilitate the coordination of care strategies and client onboarding processes.
- Work closely with healthcare providers on patient care and engage with relevant stakeholders.
- Establish and nurture partnerships with healthcare and social service teams, professionals, and organizations, taking into account the local context.
- Engage in therapeutic group activities and maintain a vigilant social awareness.
- Contribute to the overall organizational dynamics and strategic discussions.
Essential Interpersonal Skills:
- Collaborate effectively within a multidisciplinary team.
- Adapt professional conduct to a variety of situations.
- Understand and respect the boundaries within multidisciplinary collaborations.

Qualifications Required:
- A degree in Social Work is essential.

- A valid driver's license is required.
