Care Coordinator

We are seeking a highly skilled and compassionate LPN Medication Care Manager to join our team at Sunrise Senior Living. As a key member of our care team, you will be responsible for providing exceptional care and services to our residents, ensuring their physical and emotional well-being.
Responsibilities- Administer medications and treatments in a safe and organized manner
- Document medication administration and maintain accurate records
- Conduct Service and Health Updates as directed by RCD
- Participate in the development of Individualized Service Plans (ISPs) and monthly updates
- Collaborate with the community team to ensure compliance with OSHA and Occupational Health and Safety Regulation requirements
- Practice safety procedures at all times, including Personal Protective Equipment (PPE) and infection control techniques
- Previous experience working with seniors is preferred
- Ability to remain calm and composed in difficult situations
- Desire to serve and care for seniors
