Caregiver Specialist

ADMR, a leading network of proximity services to individuals, is seeking a Healthcare Professional for the association ADMR SSIAD in Freney d'Oisans.

• Conduct home visits for admissions and re-evaluations of patients' care plans, developing personalized accompaniment projects.
• Promote patients' physical and mental well-being to maintain them at home.
• Evaluate and follow up on the care team.
• Lead team meetings.
• Collaborate and coordinate with patients' entourage and home-based intervenants: IDEL, kinés, doctors.
• Ensure patient dossier follow-up and continuity of care.
• Contribute to the realization and supervision of care tasks for care assistants.
• Identify risk situations.
• Ensure respect of protocols and procedures, linked to care quality indicators, care traceability, and oral and written transmissions quality.

• Diploma in Nursing required.
• Ability to adapt to the establishment's project and respond coherently.
