- Mandataires judiciaires
- SAVS (Service d’Accompagnement à la Vie Sociale)
- SAMSAH (Service d’Accompagnement Médico-Social pour Adultes Handicapés)
- Service Domiciles Inclusifs (ex-Service Logement)
- Baux Glissants
- 3 Maisons du Grim (foyer de vie Le Petit Caillou, Les 3 Galets, La Pierre d’Orée)
- PCB (Point Conseil Budget Lyon 8)

L’Association Grim recrute un case manager à **temps plein pour un CDD d'1 an** sur le territoire de la Métropole.

CASE MANAGER

Rattachés à la Direction des Parcours, les case managers ont pour mission de soutenir les professionnels de terrain dans l’accompagnement des situations complexes au travers du développement des réseaux professionnels.

VOS MISSIONS:

- En externe_:

- Mobiliser les réseaux de partenaires et notamment les dispositifs locaux (CLSM, CPTS,)
- Assurer la gestion des parcours des personnes concernées, par un soutien aux référents de projets personnalisés, dans la conception, l’évaluation et la conduite des actions.
- Rédiger un projet d’accompagnement situation complexe dit « projets personnalisés CARE »
- Assurer le développement de partenariats utiles au parcours des personnes concernées, faire vivre le réseau existant et participer au travail de coordination.
- Accompagner 40 situations identifiées comme complexes
- Assurer le lien avec le/la psychologue prestataire pour prévoir ses interventions
- Co animer la cellule Care avec des membres de plusieurs associations.
- Réaliser des entretiens individuels auprès des personnes concernées dans le cadre des « projets personnalisés CARE »
- Participer aux ateliers au bénéfice des salariés pour prévenir les situations complexes.
- Sensibiliser le personnel aux différents dispositifs d’accompagnement du territoire d’intervention.
- En interne : _
- Assurer le lien et entretenir la dynamique de coordination entre les différents services et

établissements de l’Association, ainsi qu’avec les services mandataires ATMP69 et ASSTRA
- Représenter l’Association dans les commissions, cellules en lien avec ses activités de coordination

(Communauté 360, PTSM, DAC, CLSM )
- Renseigner et mettre à jour les indicateurs de suivis portant sur la coordination des parcours
- Animer les commissions sociales des établissements et services
- En lien avec les référents de parcours, les référents vie quotidienne et MJPM_
- Elaborer le « projet personnalisé CARE »
- Evaluer l’autonomie des personnes au moyen de la grille SERAFIN-PH
- Conseiller les intervenants dans les actions des « projets personnalisés CARE »

VOTRE PROFIL:

- Diplôme de niveau 5 et 6 travailleur social (ES, CESF, ASS, Mandataire judiciaire )
- Capacité à adapter la posture professionnelle dans le cadre de commission ouverte à plusieurs associations
- Capacité à travailler en réseau
- Travailler en autonomie
- Être force de proposition
- Capacité à fédérer
- Animation de réunion
- Capacité de rédaction

FORMALITÉS & RÉMUNÉRATION:

- Temps de travail soit 35h/semaine
- Salaire selon convention collective (cc66), à titre indicatif pour un débutant, indice 434 salaire brut - 2048€ brut (prime Ségur incluse)
- Lieu de travail : Basé sur le site des Domiciles Inclusifs - 39 avenue Sidoine Apollinaire, Lyon 9 - ils rayonnent sur l’ensemble du Département du Rhône
- Permis de conduire requis pour le poste
- Utilisation du véhicule personnel pour les déplacements (remboursement des frais professionnels)
