As an LPN, you will provide direct resident care, administer medications, and support the nursing staff in delivering high quality, resident-centered care. This position is full time. Hiring for days (7a-7p) & nights (7p-7a) ---> $10,000 Bonus for full time Pay Rates: MaxPay Program - Inquire during interview for program details.$30.00 an hour- (7a-7p)$33.00 an hour - (7p-7a) ($30.00 + $3 shift diff) _______________________________________________________________ Job Requirements: Current and active LPN license in state of TNPrevious experience in long term care is preferredStrong clinical assessment skillsExcellent communication and interpersonal skillsAbility to handle multiple tasks and prioritizeFlexibility to work various shifts including weekends and holidays if needed _________________________________________________________________ Job Responsibilities May Include: Provide direct patient care to residents following established care plansAdminister medications and treatments as prescribed, documenting accuratelyMonitor residents' health status including vital signs, physical and mental conditionAssist with admission, transfers and discharge procedures including resident assessments and documentationCollaborate with RNs, CNAs, physicians, therapists and other team members to ensure quality careMaintain accurate and timely documentation in medical recordsSupport residents with activities of daily living as neededParticipate in care planning, family conferences, and resident meetings to promote person centered careFollow infection control, safety protocols and facility policiesEducate residents and families on health management, medication use, and care proceduresAssist in training and mentoring new staff _________________________________________________________________ Why Choose Patriot Health and Rehab?
