Job Title: Healthcare Coordinator for Individuals with Disabilities
Role Overview:

Position Objective: As a Healthcare Coordinator, your primary duty is to oversee the care of individuals with disabilities, ensuring adherence to their customized support plans. You will provide care in various settings, including facilities, homes, or any location requiring medical intervention. Your role may also involve preparing necessary treatments for individuals as required. Furthermore, you will participate in project meetings and collaborate with team members to enhance health education.

  • Supervise the medical care plan for each individual
  • Create tailored support strategies
  • Accompany individuals to healthcare appointments
  • Ensure adherence to therapeutic protocols.
Company Background:

The Accommodation Service at the inclusive EANM of Sens supports 35 individuals, focusing on empowerment and skill enhancement to foster greater community inclusion. The individuals served are engaged in activities at the APAJH ESAT.

About the Federation:

The APAJH Federation is a prominent organization in the disability sector, dedicated to fostering a genuinely inclusive society through advocacy efforts. Actively participating in public discussions, the Federation operates at various governmental levels to promote its core values. It unites 92 associations across the nation, including overseas territories, and manages 171 facilities with a workforce of 4,000 employees. Overall, APAJH oversees 700 establishments and services catering to all types of disabilities and age groups.

Qualifications and Requirements:

Essential Credentials:

  • Must possess a State Nursing Diploma
  • Required to hold a B License
