GROUPE ELSAN is seeking a skilled Nurse Coordinator to join their Home Hospitalization Service (HAD) team on a permanent basis.

Key Responsibilities:

  • Coordinate the implementation of care services for patients admitted to HAD, ensuring seamless transitions and optimal patient outcomes.
  • Conduct thorough assessments of patients' situations at home, developing comprehensive nursing diagnoses and individualized care plans.
  • Collaborate with multidisciplinary teams to develop and implement care plans, promoting patient-centered care and quality outcomes.

Requirements:

  • Current Nursing Degree and valid driving license.
  • Previous experience in a similar role is highly desirable.

Job Conditions:

  • Full-time permanent position available, with opportunities for professional growth and development.
  • Home visits are required, with a focus on providing high-quality care in a patient's home environment.
  • Salary will be commensurate with experience and in accordance with the Private Hospitalization collective agreement.
