Overview
The City of Saint-Denis is committed to enhancing the quality of life for its elderly residents. Within the Department of Solidarity and Social Development, the Seniors division of the Public Social Welfare Center (CCAS) integrates various services aimed at supporting seniors, including Home Assistance, Autonomy Residences, Sunlight Homes, and a dedicated Seniors' House focused on leisure, prevention, and combating social isolation. Since 2018, the CCAS has also been piloting a specialized gerontology coordination service tailored to the needs of the community, alongside a modular reception platform for the elderly.
In collaboration with a range of internal and external stakeholders, the gerontology coordination service is tasked with evaluating the complex situations of elderly individuals with reduced autonomy living independently. Its primary function is to connect seniors with primary healthcare, social and medical-social support, Home Assistance services, and, when necessary, facilitate transitions to alternative living arrangements such as nursing homes. Additionally, it plays a vital role in preventive initiatives.
Key Responsibilities
- Serve as the primary contact for the integrated senior service desk, developed in partnership with the Coordination Support Team and the Departmental Council.
- Conduct comprehensive early assessments in accordance with agreements established with the Departmental Council.
- Oversee the processing of applications for entry into Autonomy Residences or Sunlight Homes.
- Foster a unified gerontological understanding among internal and external stakeholders and provide technical assistance for complex resident cases.
- Collaborate with partners to implement action plans or redirect individuals to the most appropriate services as needed.
- Inform individuals and partners regarding issues related to reported elderly individuals.
- Ensure coordinated actions for reported individuals by the interdisciplinary team of the Croizat Residence and the dedicated team of the Laurier Noble Nursing Home.
- Manage the dedicated team within the division (social worker and secretary).
- Supervise the activities, occupancy rates, and budget oversight of the establishment.
- Lead initiatives for project submissions.
Candidate Profile
- Proficient in evaluating social circumstances.
- In-depth knowledge of the elderly demographic, relevant legislation, and support frameworks.
- Strong project management capabilities.
- Exceptional written communication skills for documentation and social assessments.
- Robust interpersonal skills for engaging with teams, clients, families, and partners.
- Demonstrated availability, active listening, and empathy.
- Ability to adapt and manage stress effectively.
- Dynamic and capable of proposing innovative ideas.
- Competence in managing meetings and coordinating actions.
- Familiarity with team management tools, planning, facilitation, communication, and skills development.
Qualifications
- Possession of a social work diploma and/or a level II diploma.
- Preferred experience in the medical-social sector.
- Desired management experience.
