Job Title: SAPHA Pathway Coordinator
About the Organization

Your Key Responsibilities
- Providing a warm welcome, information, and guidance while conducting thorough assessments
- Assessing the appropriateness and type of long-term assistance required
- Facilitating follow-up for individuals and families to enhance autonomy and integration through structured interventions
- Creating integration plans for RSA beneficiaries via contractual agreements
- Participating in evaluations concerning risk or danger in child protection and vulnerable adult cases
- Leading or engaging in collective initiatives and informational sessions
- Attending technical meetings within the service
- Assisting in the training of students and the onboarding of new staff members
Qualifications
The ideal candidate should possess the ability to conduct both individual and group interviews, evaluate risk factors in personal or family situations, manage timelines effectively, produce comprehensive reports, collaborate with other services, and exhibit autonomy, availability, teamwork, adaptability, and a solid understanding of social action policies, relevant services, and legislation. Experience working with disabled populations is preferred.
Eligibility Criteria
This position is open exclusively to individuals holding specific qualifications such as specialist educator, social service assistant, family and social economy advisor, early childhood educator, technical educator, nurse, educator monitor, and social and family intervention technician. A valid driver's license is mandatory. This is a full-time role based within the Departmental House of Solidarity.
