Job Summary

As a Geriatric Care Coordinator, you will be responsible for ensuring the quality of gerontological care for residents by coordinating healthcare professionals and adjusting prescriptions to meet their needs.

Main Responsibilities
  • Care Project Development: Develop, implement, and evaluate care projects in collaboration with the team and external physicians.
  • Resident Admissions: Provide input on admissions to ensure new residents' needs align with the facility's capabilities.
  • Dependency Assessment: Assist in evaluating residents' dependency levels in partnership with the care staff.
  • Care Organization: Coordinate healthcare professionals and ensure continuous care.
  • Geriatric Best Practices: Promote best geriatric practices and make recommendations for high-quality care.
  • Prevention and Safety: Identify risk behaviors, develop risk management protocols, and implement prevention strategies.
  • Prescription Adaptation: Ensure prescriptions align with geriatric needs in collaboration with treating physicians.
  • Care Records Management: Develop and maintain accurate care records.
  • Training and Information: Participate in healthcare professional training and provide information to residents and families.
  • Annual Medical Activity Report: Prepare a detailed annual report on care provision and residents' health status.
  • Partnerships: Support collaboration with external healthcare providers, hospitals, home care services, and geriatric networks.
Required Skills
  • Solid knowledge of geriatric pathologies
  • Proficiency in assessment tools (AGGIR, PATHOS, NPI, etc.)
  • In-depth understanding of current regulations

  • Ability to work in teams and share knowledge
  • Strong management and meeting facilitation skills
  • Sense of responsibility, dynamism, rigor, and autonomy
Qualifications
  • Medical degree with experience in a clinical or hospital setting
  • Hold a specialization in gerontology or geriatrics, or commit to completing training within three years of employment
