Description du poste
Le coordinateur en activité physique adaptée (APA) aura pour mission de développer et structurer la coordination de l'APA dans la prise en charge de l'obésité sur le territoire des Centres Spécialisés de l'Obésité (CSO) d'Île-de-France, en lien avec les équipes médicales et les partenaires.
Ses missions incluent :
• Organisation et suivi des parcours patients en APA.
• Animation et développement du réseau de partenaires (mise en relation, participation à la formation, conventionnement avec les CSO IDF).
• Intégration de l'APA dans les parcours de soins existants (adulte, enfant, pré et post-chirurgie bariatrique).
Activités :
Cartographie des lieux de pratique d'APA en Île-de-France (Maisons Sport-Santé, clubs sportifs, EAPA, etc.).
Développement des partenariats et renforcement du maillage territorial des Maisons Sport-Santé (MSS).
Conception et mise en place d'actions de formation à destination des Enseignants en APA (EAPA).
Amélioration de l'adressage des patients :
o Structuration et animation de la filière APA / Obésité.
o Développement d'outils pratiques facilitant l'orientation des patients par les professionnels de santé.
Soutien à la création d'outils favorisant la promotion et l'accès à l'APA (solutions digitales ou autres supports).
Mise en place d'outils de suivi et d'évaluation pour monitorer l'évolution des patients.
