Case Manager

Overview

Merakey is seeking a Case Manager to join our adult residential program in Nazareth, PA. The Case Manager functions as the primary liaison to all community hospitals and other social services, mental health centers, intensive case-management units, etc., with regard to any referral for admission and/or discharge planning and follow-up contacts for our 10 residents.

Responsibilities

• Coordinate the referral process for potential admissions.
• Conduct pre-admission screenings.
• Ensure all required paperwork is completed for new admissions.
• Participate in Recovery Team meetings.
• Provide coordination updates.
• Complete all other tasks as assigned.

Earn $19.58 per hour.
