About the Role

VSGP is seeking a highly skilled and compassionate Senior Gerontology Coordinator to join our team. As a key member of our organization, you will be responsible for coordinating gerontology services and programs for seniors in our community.

Key Responsibilities

  • Provide gerontology services and support to seniors in our community, including assessment, planning, and coordination of care.
  • Develop and implement gerontology programs and services to meet the needs of seniors, including education, support groups, and advocacy.
  • Collaborate with healthcare providers, social services, and community organizations to ensure seamless care coordination.
  • Conduct home visits and assessments to identify seniors' needs and develop personalized care plans.
  • Provide education and support to seniors and their families on gerontology-related topics, including aging, dementia, and caregiving.
  • Develop and maintain relationships with community partners to ensure access to resources and services for seniors.
  • Participate in quality improvement initiatives to enhance gerontology services and programs.

Requirements

  • Bachelor's degree in a related field, such as gerontology, social work, or healthcare administration.
  • Minimum 2 years of experience in gerontology, social services, or a related field.
  • Excellent communication and interpersonal skills, with the ability to work with diverse populations.
  • Strong assessment and planning skills, with the ability to develop and implement effective care plans.
  • Ability to work independently and as part of a team, with a strong commitment to customer service.
  • Valid driver's license and reliable transportation.
