Contribute to a new model of elderly care

Permanent contract – Full-time or part-time – Remote work possible

As part of an innovative medical project, we are actively seeking general practitioners, geriatricians, and emergency physicians. Their role will be to contribute to the design and implementation of a new model of geriatric care, fully focused on the management of elderly patients with multiple chronic conditions.

Project background

Many healthcare professionals share the same observation today:

elderly patients with multiple comorbidities often follow fragmented and reactive care pathways, leading to frequent — and sometimes avoidable — hospitalizations, as well as significant dissatisfaction for both professionals and patients.

This project aims to build a more anticipatory, coordinated, and efficient model of elderly care, based on:

  • anticipation rather than crisis management,
  • coordination of stakeholders rather than isolated interventions,
  • thoughtful use of digital tools to support medical time,
  • preservation of autonomy and quality of life for older adults.

The project is built on solid medical, organizational, and medico-legal foundations and follows a structured, territory-based approach.

Your role

As a physician involved in the project, you will actively contribute to building and deploying the model through:

  • Geriatric tele-expertise

Clinical support for frontline professionals (general practitioners, private nurses, nursing homes, and medico-social structures) in managing complex, multi-morbid situations.

  • Targeted teleconsultations

Medical interventions for unplanned or high-risk situations, integrated into coordinated care pathways with a clear objective: preventing care disruptions and avoidable hospitalizations.

  • Care pathway coordination

Contribution to innovative territorial systems (including "nursing home without walls" approaches), aimed at securing care for frail patients at home or in institutions.

  • Structured collaborative work

Participation in regular multidisciplinary meetings, peer discussions, collective reflection on practices, and continuous improvement of the care model.

  • General practitioner or geriatrician registered with the French Medical Council

  • All profiles welcome: early-career physicians, experienced practitioners, and post-retirement professionals
