OBJECTIVE

As a Social Support Coordinator, your primary role will be to provide essential assistance to caregivers and patients navigating the complexities of healthcare and social services.

KEY RESPONSIBILITIES

  • Support caregivers in managing the necessary arrangements related to end-of-life services.
  • Welcome, listen to, inform, guide, and provide emotional support to patients and their families.
  • Facilitate the transition of patients back to their homes by assisting with the completion of APA applications, identifying financial aid options, and creating personalized support plans.
  • Compile and manage social assistance application files, ensuring their progress through administrative channels.
  • Develop a tailored social support project in collaboration with the patient and monitor its implementation.
  • Assess and diagnose social needs, collaborating with the healthcare team to identify the most suitable solutions based on the patient's and family's requirements.
  • Address issues related to patient care by helping them access their rights concerning social protection, health insurance, and other relevant services.
  • Direct patients and/or caregivers to medical and social partners to ensure seamless care continuity between healthcare facilities and community services.
  • Identify and report vulnerable or at-risk individuals, taking appropriate actions to safeguard their well-being.
  • Coordinate transitions to specialized facilities by preparing admission requests and providing information on available financial assistance.
  • Ensure that patients have access to their general and specific rights based on their individual needs and circumstances.
