Key Responsibilities:
- Ensure Continuity of Care: Coordinate and organize the availability of healthcare services to ensure seamless care for residents.
- Healthcare Provider Coordination: Work with healthcare providers to develop and implement care plans that meet the unique needs of residents.
- Care Project Development: Participate in the development of the facility's care project, ensuring that it aligns with the organization's mission and values.
- Collaboration with Medical Team: Work closely with the medical team to assess residents' needs and provide necessary care, promoting good practices in healthcare.
- Quality Improvement: Identify areas for improvement and implement changes to enhance the quality of care provided to residents.
Requirements:
- Education: Degree in Medicine
- Additional Qualifications: Additional qualification in medical coordination is a plus
- Preferred Qualifications: Driver's License preferred
