Overview

The Ville de Saint-Denis is committed to enhancing the quality of life for its elderly residents. Within the Department of Solidarity and Social Development, the Seniors division of the Public Social Welfare Center (CCAS) provides a range of services tailored for seniors, including Home Assistance, multiple Autonomy Residences, Sunlight Homes, and the Seniors' House, which focuses on leisure activities, prevention, and combating social isolation. Since 2018, the CCAS has also been pioneering a gerontology coordination service designed specifically for the community, alongside a modular reception platform dedicated to elderly care.

In collaboration with various stakeholders, the gerontology coordination service plays a crucial role in evaluating the complex needs of elderly individuals with diminished autonomy living independently. Its mission includes connecting seniors to primary healthcare, social and medical support, Home Assistance services, and guiding them towards alternative living arrangements when necessary, such as nursing homes. Additionally, the service contributes to preventive initiatives.

Key Responsibilities

  • Serve as the primary contact for the integrated senior service desk, developed in collaboration with the Coordination Support Team and the Departmental Council.
  • Conduct comprehensive initial assessments in accordance with agreements established with the Departmental Council.
  • Oversee the processing of applications for entry into Autonomy Residences or Sunlight Homes.
  • Foster a unified gerontological culture among internal and external stakeholders and provide expert support for complex resident cases.
  • Collaborate with partners to create and implement action plans or redirect individuals to the most appropriate services as necessary.
  • Provide information to individuals and partners regarding concerns related to elderly individuals.
  • Ensure effective coordination of actions taken for reported individuals by the interdisciplinary team of the Croizat Residence and the dedicated team of the Laurier Noble Nursing Home.
  • Manage the dedicated team within the division, including social workers and administrative staff.
  • Supervise the activities, occupancy rates, and budget management of the establishment.
  • Lead initiatives for project submissions.

Candidate Profile

  • Strong ability to assess social situations.
  • In-depth knowledge of the elderly population, relevant legislation, and support systems.
  • Proficient in project management.
  • Exceptional written communication skills for documentation and social assessments.
  • Excellent interpersonal skills for engaging with teams, patients, families, and partners.
  • Demonstrated availability, active listening, and empathy.
  • Ability to adapt and manage stress effectively.
  • Dynamic approach with a capacity to propose innovative ideas.
  • Competence in managing meetings and coordinating actions.
  • Familiarity with team management tools, planning, facilitation, communication, and skills development.

Qualifications

  • Possession of a social work diploma and/or a level II diploma.
  • Experience in the medical-social sector is preferred.
  • Management experience is desirable.
